Light Whispers Client Policies
1. Client Intake & Consent
Policy: All clients must complete a comprehensive intake form and sign informed consent before receiving any services.
Procedure:
Clients complete a medical history form pre or during the first visit.
Clients sign an informed consent form and a waiver of liability.
It is the client’s responsibility to inform the practitioner of any changes to their medical file for subsequent visits
2. Privacy & Confidentiality
Policy: All client information is kept confidential in compliance with privacy legislation.
Procedure:
Physical files stored in locked cabinets and digital files in secure, encrypted software.
Only authorized practitioner may access client records.
Client information will not be disclosed without written consent.
3. Bookings & Cancellations
Policy: Clients must provide 24 hours’ notice for cancellations or rescheduling.
Procedure:
Communicate the policy during intake and on appointment reminders.
Charge a cancellation fee of $50 for late cancellations.
Allow online or phone-based scheduling.
4. Payment & Billing
Policy: All services must be paid at the time of booking unless otherwise arranged.
Procedure:
Accept cash, credit cards, and bank transfers.
Receipts and invoices provided upon request.
5. Client Feedback & Complaints
Policy: Feedback & complaints can be provided via email.
Procedure:
All feedback/complaints to be emailed to kaz@lightwhispers.com.au
Response to emails will be within 5 business days.
